North Highland ESOP Holdings and related defendants agreed to pay $2,375,000 to settle a class action lawsuit alleging breaches of fiduciary duty and other violations under the Employee Retirement Income Security Act. Data Breach · Claims Open

Current Status

Claims are open. Claim Forms must be submitted online by November 5, 2026, or mailed with a postmark no later than that date; the notice states a date rather than a cutoff time. The deadline to exclude yourself or to object is earlier, on October 21, 2026, and the final approval hearing is scheduled for 10:00 a.m. on November 20, 2026 in Denver — so the court has not yet decided whether to approve the settlement, and no payment date had been announced as of August 30, 2026. If YES Communities mailed you a postcard notice with a Claim Number and PIN on it, the action to take now is to file a Claim Form. Nothing arrives automatically here: the notice says a class member who does nothing gets no money and no credit monitoring, and still releases their claims.

What Changed Recently?

Notice has gone out to the settlement class and the claim window is now running toward its November 5, 2026 close. The case is captioned O'Leary v. YES Communities, LLC, Case No. 1:25-cv-00692-PAB-NRN, in the U.S. District Court for the District of Colorado. Five separate class actions were filed over the same incident beginning in March 2025; the court consolidated four of them in April 2025, a consolidated complaint followed in May 2025 covering all five, and the parties reached terms after an October 2025 mediation.

According to the court-authorized notice, YES Communities became aware of anomalous activity on its computer network on or about December 11, 2024, involving certain files encrypted by ransomware. A forensic investigation determined that between December 9 and 11, 2024 the company experienced a ransomware attack that resulted in unauthorized access to its systems and the theft of unencrypted Private Information belonging to 10,675 individuals in the United States or its territories.

The categories involved are unusually broad for a housing company, and they are what signal that the class reaches employees as well as residents: the notice lists name, Social Security number, driver's license number, passport number, state ID card number, financial and credit or debit account information, visa and work eligibility documentation, employee identification or tax ID number, non-US national identification number, and date of birth. YES Communities began sending notification letters on or about February 24, 2025.

The consolidated complaint alleged that the company failed to safeguard that information and that it waited more than two months after the attack to begin notifying people. YES Communities denies any wrongdoing, expressly disclaims any fault or liability, and the court has not decided who is right. The parties settled to avoid the cost and risk of continued litigation.

Who Qualifies?

The Settlement Class is all living individuals in the United States who were sent notice by YES Communities that their Private Information was impacted in the Data Incident. The mailed notice is the gate: eligibility turns on having been notified, not on whether you rented, applied or worked there, and the notice tells recipients directly that receiving it means they are in the class.

YES Communities owns manufactured home communities across 23 states, so the class spans residents, former residents, applicants for housing and current and former employees — anyone whose information was in the files the attacker took. The class is nationwide and is not limited by state.

Excluded are the directors and officers of YES Communities, governmental entities, and the judge assigned to the case along with that judge's immediate family and court staff. If you are unsure whether you were notified, the contact page on the official settlement website is the place to ask.

Data Breach · Claims Open

Current Status

Claims are open. A Claim Form must be submitted online by November 6, 2026, or mailed so that it is postmarked by that date; the court-approved notice gives the date without stating a filing-time cutoff or a timezone. The deadline to opt out or to object is more than two weeks earlier, October 22, 2026. The court has authorized notice to the class but has not decided whether to approve the settlement: the final approval hearing is scheduled for November 11, 2026 at 10:00 a.m. Eastern Time in Manhattan, and the notice warns the date and time may change without further notice. No payments have been issued and no payment date has been announced. If you were sent a notice about the SitusAMC data incident, the step to take now is to file — the one year of credit monitoring arrived automatically, but every cash payment in this settlement requires a Claim Form, so a class member who does nothing receives no money and is still bound by the release.

What Changed Recently?

The court authorized notice to the settlement class and the claim process opened, with a November 6, 2026 deadline and an official settlement website now live. That is the development here. It also resolves the open question this site flagged in July, when the parties had asked for preliminary approval but the judge wanted more detail on the notice plan and the administration budget first: at that point there was no settlement website, no claim form and no deadline. All three now exist.

The consolidated case is In re SitusAMC Holdings Corporation Data Breach Litigation, No. 1:25-cv-09748, in the U.S. District Court for the Southern District of New York.

SitusAMC denies all material allegations in the complaint and specifically denies that it failed to protect any data, had inadequate data security, was unjustly enriched, breached any duty or contract, or violated any consumer protection statute. The notice states that the court did not decide in favor of either side and that the parties agreed to settle to avoid the costs, risks and uncertainties of continuing the case. No court has found SitusAMC liable.

What Is This Settlement About?

SitusAMC Holdings Corporation is a New York-based technology and services provider for the real estate finance industry, which is why the people affected here are mostly not its own customers. Banks and lenders send SitusAMC borrower and asset data to process; when that data was taken, the individuals behind it were customers of those institutions.

According to the notice, on or about November 12, 2025 SitusAMC discovered that it had been the victim of the data incident, which involved unauthorized access to and exfiltration of information from its systems. What was taken varied person by person, but the notice lists names, dates of birth, Social Security or individual taxpayer identification numbers, driver's license numbers, state identification numbers or other government-issued identifiers, financial account numbers, medical records, and health insurance policy numbers.

SitusAMC notified its clients on November 22, 2025 and confirmed the incident publicly about two days later, saying no ransomware was deployed, its operations were not disrupted, and it had notified and was cooperating with the FBI. Customers of major bank clients — including JPMorgan Chase and Citi, and reportedly Morgan Stanley — were among those whose data was potentially involved. Mailed notices to affected individuals began going out on January 28, 2026 with an offer of complimentary credit monitoring.

Current Status

There is no claim form: class members identified in Sun Country's records will be paid automatically if the court approves the settlement. The U.S. District Court for the District of Minnesota certified the class and granted preliminary approval on August 11, 2026. The deadline to object is October 30, 2026, and the deadline to challenge Sun Country's data is November 6, 2026. The final approval hearing is set for November 17, 2026 at 1:00 p.m. Central Time by Zoom. No payment date had been announced as of September 30, 2026.

What Changed Recently?

Sun Country, Inc., the Board of Trustees of the Sun Country, Inc. 401(k) Profit Sharing Plan and its members agreed to pay $1,550,000 to resolve Smith v. Sun Country, Inc., et al., a class action over employer contributions for pilots who left for military service. The court certified the class and preliminarily approved the deal on August 11, 2026, and notice went out to the pilots identified in Sun Country's data.

The lawsuit, filed on February 27, 2024, alleges that Sun Country did not make the 401(k) contributions the Uniformed Services Employment and Reemployment Rights Act (USERRA) requires for a returning servicemember's time away, and that the plan's fiduciaries breached their duties under ERISA by not requiring those contributions. Sun Country pilots receive a nonelective employer contribution based on their pay, so leave that was not credited meant a smaller retirement account. Sun Country and the other defendants deny wrongdoing and any liability, and the settlement is not an admission.

While producing data in the case in September 2025, Sun Country identified that its payroll calculations for returning pilots had not consistently matched the method it intended to use and disclosed that to the court and plaintiffs' counsel. The company says it switched to a revised calculation on October 1, 2025. The parties settled at a second court-supervised settlement conference on June 10, 2026, after a first one in August 2025 ended without a deal.

A separate case produced the Alaska Airlines pilot military leave settlement, which pays pilots for vacation that did not accrue during military leave.

Who Qualifies?

The class covers current and former Sun Country pilots who participated in the Sun Country, Inc. 401(k) Profit Sharing Plan and who:

  • completed a period of qualified military service between July 21, 2011 and December 31, 2025, after becoming a Sun Country employee;
  • returned from that leave and were re-employed at Sun Country; and
  • did not receive a plan contribution for that service at the rate USERRA requires — the rate they would have earned but for the leave, or, when that rate is not reasonably certain, their average pay rate over the 12 months before the leave.

Beneficiaries of those pilots are also class members. Plan fiduciaries named as defendants, their families and beneficiaries, and anyone who already settled or lost the same claims are excluded. Class counsel identified about 131 current and former pilots in Sun Country's data. A pilot who received the notice without asking for it has already been identified as a class member.

How Much Can You Get?

Payments are individual. Under the proposed plan of allocation, each pilot's share is based on the contributions that plaintiffs' expert calculated should have been made for their military leave, plus the investment earnings those contributions would have produced, minus any contributions Sun Country actually made. The notice says the settlement administrator can share the underlying data but cannot yet give an estimate for any one person.

Twin Cities Pain Clinic Data Breach Class Action Settlement

September 18, 2026 12:58 PM

Individuals whose private information unauthorized individuals accessed or acquired in the July 2025 data incident at Twin Cities Pain Clinic may be eligible to claim up to $4,500 and two years of credit monitoring from a class action settlement .

Twin Cities Pain Clinic agreed to settle a class action lawsuit alleging it failed to adequately protect sensitive personal information, which may have included names, dates of birth, contact details, Social Security numbers, health insurance information and medical records.

Who can file a claim?

Class members are all individuals in the United States whose private information the July 2025 data incident at Twin Cities Pain Clinic may have exposed.

How much can class members get?

Eligible class members will receive the following benefits:

  • Extraordinary losses: Individuals who experienced actual, documented out-of-pocket losses due to the data incident can claim up to $4,500 for extraordinary losses, such as identity theft or fraud. The losses must have occurred between July 9, 2025, and Nov. 9, 2026.
  • Ordinary losses: Class members who did not incur extraordinary losses may claim up to $450 for ordinary losses, such as credit monitoring fees, costs to replace IDs and postage. The losses must have occurred between July 9, 2025, and Nov. 9, 2026.
  • Flat cash payment: Individuals who do not claim extraordinary or ordinary losses can claim a one-time payment expected to be $55.
  • Credit monitoring: All class members are eligible for two years of CyEx Financial Shield Complete credit monitoring, which includes $1 million in financial fraud insurance and monitoring for identity theft, unauthorized financial transactions and high-risk activity.

The settlement caps the total cash payments to all class members at $385,000. If the total amount class members claim exceeds this cap, the settlement administrator will reduce each claimant’s payment proportionally so the total does not exceed $385,000.

Data Breach · Claims Open

Current Status

Claims are open. The deadline to file is November 9, 2026, online or by mail with a postmark no later than that date. The deadlines to opt out and to object land on the same November 9, 2026 rather than weeks earlier, which is unusual and means the decision to stay in the class does not close ahead of the money. The Court has set a Final Approval Hearing for December 10, 2026 at 2:00 p.m. Eastern Time, and the settlement has not been approved yet. No payment date has been announced: the Notice says settlement payments are distributed if the Court grants final approval and after any appeals are resolved. If a notice reached you, it carries the Login ID and PIN that open the online claim form.

What Changed Recently?

The settlement is newly open to claims. Valley Oaks Health, Inc. has agreed to resolve In re: Valley Oaks Data Incident Litigation, Case No. 79D01-2403-PL-00032, in the Superior Court No. 1 for Tippecanoe, Indiana, and the Court has authorized notice to the class. Simpluris is administering it, and the claim portal, the long-form notice, the claim form and the settlement agreement are all live on the official settlement website.

Valley Oaks denies that it did anything wrong, and the Court has not decided who is right. The Notice states that the parties agreed to settle to avoid the costs and risks, disruptions and uncertainties of continuing the litigation, and that the Court did not decide whether the plaintiffs or the defendant are right. No finding of liability has been made against the company.

Who Qualifies?

The Court defined the Class as all persons whose Private Information was potentially compromised as a result of the Incident and who are included on the Settlement Class List. Both halves of that definition matter, and the second one does most of the work: membership is not a judgment call about whether your information was in the files, it is a question of whether the administrator’s list has your name on it. The Login ID and PIN printed on the mailed notice are the practical signal, and the Notice says Valley Oaks’ records indicate a recipient is a class member entitled to benefits.

The Notice also says class members may have received an earlier notice directly from Valley Oaks, separate from the court-authorized one. That earlier letter is not the claim notice and does not carry the credentials the claim form asks for.

Excluded from the Class are the Judge assigned to evaluate the fairness of the Settlement, Court personnel and their immediate family members; Valley Oaks, any entity in which Valley Oaks Health, Inc. has a controlling interest, any parent, subsidiary or entity it controls, and the affiliates, legal representatives, attorneys, heirs, predecessors, successors and assigns of Valley Oaks; and anyone found by a court of competent jurisdiction to be guilty under criminal law of initiating, causing, aiding or abetting the criminal activity behind the Incident, or who pleads nolo contendere to such a charge.

The lawsuit alleges that during the June 2023 targeted cyberattack on Valley Oaks’ computer systems, certain files containing private information were accessed. Our explainer on how data breach class actions work covers what these cases typically pay and what a release costs.

How Much Can You Get?

There is one branching decision on the claim form, and one benefit that sits outside it. The cash options are alternatives:

  • Alternative Cash Payment — $40. A one-time payment claimed instead of any other payments. The Notice says no proof or explanation is required.
  • Ordinary Out-of-Pocket Losses — up to $500. Actual, documented, unreimbursed expenses caused by the Incident and incurred between June 8, 2023 and November 9, 2026.
  • Extraordinary Out-of-Pocket Losses — up to $5,000. Money lost to identity theft or fraud, in the same June 8, 2023 to November 9, 2026 window, where the loss is not already covered by the ordinary tier.

Data Breach · Claims Open

Current Status

Claims are open. The deadline to submit a Claim Form is November 9, 2026, at 11:59:59 p.m. Central for an online submission, or with a November 9, 2026 postmark by mail. The final approval hearing is set for December 14, 2026 at 11:00 a.m. Central at the Tuscaloosa County Courthouse in Tuscaloosa, Alabama. No final approval order has been entered, and no payment date has been announced.

A second date lands first and is easier to miss. October 12, 2026 is the deadline both to exclude yourself and to object — almost a month before the claim window shuts. Opting out is the only way to keep the right to sue Peco Foods separately over this breach, and that door closes first.

What Changed Recently?

The claim window opened this fall on a breach that happened nearly three years ago. Peco Foods Inc. is one of the largest poultry processors in the United States, headquartered in Tuscaloosa, Alabama, and the settlement concerns a data breach that occurred on or around December 4, 2023, in which the complaint alleges an unauthorized third party accessed personal identifiable information belonging to the plaintiffs and the settlement class.

Two dates in the record are worth putting side by side. The incident is dated on or around December 4, 2023. The Notice of Data Breach that defines who is in the class went out on or around July 24, 2024 — roughly seven and a half months later. The settlement documents do not explain the gap, and the notice does not publish how many people were sent a letter, so neither figure should be assumed from the fund size.

Nine named plaintiffs brought the case, which is captioned Ryan, et al. v. Peco Foods Inc., Case No. 63-CV-2025-900067, in the Circuit Court of Tuscaloosa County, Alabama. It settled for a $950,000 common fund before any class was certified for trial.

Peco Foods denies the claims. The notice states that the company denies it did anything wrong and denies that the incident caused any harm to the representative plaintiffs or to the settlement class members whose data may have been accessed, and that no court or other judicial entity has made any judgment, finding or determination that Peco has any liability for these claims or did anything wrong. The court has not decided who is right; both sides agreed to settle to avoid the cost and risk of a trial.

Who Qualifies?

The settlement class is people who reside in the United States and received written notice from Peco Foods that their personal identifiable information may have been compromised in the breach. The class definition the settlement website publishes ties that to a specific mailing: individual U.S. residents whose information was accessed or acquired in the breach that is the subject of the Notice of Data Breach Peco sent on or around July 24, 2024.

Peco's own records decide who that is, so in practice the question is whether a notice letter arrived.

Excluded from the class are the judge presiding over the litigation, that judge's staff and immediate family; Peco's officers, directors, members and shareholders; anyone who timely and validly requests exclusion, along with their successors and assigns; and any person found by a court of competent jurisdiction to be criminally guilty of initiating, causing, aiding or abetting the breach, or who pleads nolo contendere to such a charge.

Current Status

Claims are open. Notices were mailed and emailed on September 8, 2026, and the deadline to submit a Claim Form is November 9, 2026, whether you file online or return a paper form by mail or email postmarked by that date. The same date is the deadline to opt out or to object. The court has not yet decided whether to approve the settlement: King County Superior Court granted preliminary approval on July 23, 2026, and the final approval hearing is set for February 19, 2027 at 8:30 a.m. before Judge Nikole Hecklinger. No payments have been issued and no payment date has been announced — the Notice says checks are mailed after final approval and after any appeals and other reviews are complete. If a notice arrived with a CPT ID and passcode, the action to take now is to file before November 9.

What Changed Recently?

The claim window opened this month. King County Superior Court granted preliminary approval on July 23, 2026 and authorized the settlement administrator, CPT Group, Inc., to issue the Notice and Claim Form. Those went out on September 8, 2026, which started the 60-day window Class Members have to file, exclude themselves, or object.

The underlying case is Pierre v. Covenant Transport, Inc., Case No. 24-2-13005-5 SEA, filed in King County Superior Court in Washington on June 11, 2024. The complaint alleges that Covenant Transport violated RCW 49.58.110, the job-posting provision of Washington's Equal Pay and Opportunities Act, by advertising open team driver positions without disclosing the wage scale or salary range, or a general description of the benefits and other compensation being offered. That statute requires employers with 15 or more employees to put pay and benefit information in the job advertisement itself.

Covenant Transport denies the allegations and denies that it is or can be held liable for the claims in the lawsuit. The Notice states that the court has made no determination on the merits and has not decided whether the company violated the law, and the Settlement Agreement provides that settling is not an admission of liability or wrongdoing. The parties agreed to resolve the case to avoid the uncertainty, risk and expense of continued litigation.

Who Qualifies?

The settlement class is narrow and precisely drawn: everyone who submitted a job application online to, and for, a team driver position with Covenant Transport in the State of Washington between January 1, 2023 and June 17, 2024.

Each element matters. The application had to be submitted online rather than on paper or in person. It had to be for a team driver role specifically, not another position at the company. And it had to be for a position in Washington. Applications outside that class period fall outside the settlement even if the posting looked the same.

You did not need to be hired, interviewed or contacted, and you did not need to lose anything. The claim is about what the posting disclosed, not about the outcome of the application. Covenant Transport identified approximately 275 Class Members from its own records, so the practical test is whether a notice was sent to you. That is a small class by the standards of these cases, which is part of why the individual estimate is a four-figure sum rather than a token payment. If you are unsure whether you are covered, the settlement website's contact page is the route to ask the administrator.

How Much Can You Get?

Covenant Transport agreed to pay a Minimum Settlement Fund of $288,125 and a Maximum Settlement Fund of $412,500, with the amount actually owed set by how many Class Members file valid claims. Everyone who files a timely, valid claim receives an equal share of the Net Settlement Fund — what is left after the court-approved deductions described below. There are no tiers and no documented-loss category; every valid claim is worth the same as every other one.

Health Privacy · Claims Open

Current Status

Claims are open. A claim has to be filed by November 9, 2026; the notice gives no time of day, so file early. The deadline to exclude yourself is a month earlier, October 9, 2026. The Circuit Court of Cole County, Missouri has scheduled a final approval hearing for December 1, 2026. No final approval order has been entered and no payment date had been announced as of September 24, 2026.

What Changed Recently?

The Court authorized notice of a proposed settlement in Doe v. Boone Health, Inc., Case No. 22AC-CC07646, in the Circuit Court of Cole County, Missouri, which opened the claim period and set the December 1, 2026 final approval hearing. The defendants are Boone Health, Inc., doing business as Boone Health, and CH Allied Services, Inc., doing business as Boone Health and Boone Hospital Center, the Columbia, Missouri health system.

The lawsuit alleges that Boone Health used third-party tracking technologies, such as the Meta Pixel and Google Analytics, on its public website and on its patient portal, and that those tools transmitted patients' information to third parties without their consent. Boone Health denies the allegations and denies any wrongdoing, and the Court has not decided who is right. The settlement resolves the claims without a finding either way.

Who Qualifies?

The class covers current and former Boone Health patients who reside in Missouri and who logged into the Boone Health Patient Portal between April 1, 2021 and December 5, 2022.

Three conditions all have to be met: being a Boone Health patient, living in Missouri, and actually logging into the portal during that window. Visiting Boone Health's public website without signing into the portal is not enough, and neither is being a patient who never used the portal. Patients who were mailed a notice with a Login ID and password are the people the settlement identified from its records.

How Much Can You Get?

The settlement offers two benefits, and they are claimed in different ways:

  • Cash — $20. A flat payment for each class member who files a valid claim. The settlement describes it as a set amount per claim rather than a share of a fixed pool, so the number of people who file does not change it.
  • CyEx Privacy Shield Pro. An identity and privacy protection product offered to class members. Enrollment runs separately from the cash claim, with its own instructions that the settlement says are sent by email.

Nothing is paid until the Court grants final approval and any appeals are resolved.

What Proof or Notice ID Is Required?

No receipts or medical records are required. Filing online does require the Login ID and password printed on the notice mailed to class members, which makes this a proof-required settlement even though no receipts are needed: a patient who never received the notice, or who threw it away, cannot get into the online claim form without those credentials.

A paper claim form is available as an alternative to the online form. The official settlement website's contact page is the route for anyone who needs the administrator's help with a lost Login ID or password.

What Is the Deadline?

  • October 9, 2026 — opt-out. The last day to exclude yourself from the settlement and keep the right to bring your own claim against Boone Health.
  • November 9, 2026 — claims and objections. The last day to file a claim, and the last day to object to the settlement.
  • December 1, 2026 — final approval hearing. The Court decides whether to approve the settlement.

The notice gives dates only, without a time of day or time zone. Filing well before November 9 avoids any question about a late-evening cutoff.

Securities · Pending — Automatic Payment

Current Status

There is no claim form and no claim deadline. Eligible class members are paid automatically if the Court approves the settlement. The dates that matter are procedural: written objections, and any notice of intention to appear at the hearing, must be received by November 9, 2026. The settlement hearing is scheduled for November 23, 2026 at 10:00 a.m. before Vice Chancellor J. Travis Laster in the Delaware Court of Chancery in Wilmington, and the Court may hold it remotely. No final approval has been granted and no payment date had been announced as of October 6, 2026.

What Changed Recently?

The parties signed the settlement stipulation on September 11, 2026, and on September 16, 2026 the Court entered a scheduling order directing that notice go to the class and setting the November 23 hearing. The notice is dated September 24, 2026. The settlement resolves Plymouth County Retirement Association v. Restaurant Brands International Inc., C.A. No. 2024-1030-JTL, for $18.2 million in cash, funded through the defendants’ insurance coverage and contributions.

The deal came after the Court denied the defendants’ motions to dismiss on July 22, 2025, followed by fact discovery in which the defendants and third parties produced more than 258,000 pages of documents. A mediator’s recommendation led to an agreement in principle on May 18, 2026. The defendants deny all wrongdoing and maintain the $9.55 merger price was fair; the settlement is not an admission of liability.

Who Qualifies?

The class covers registered holders and beneficial owners of Carrols Restaurant Group common stock who received, or had the right to receive, $9.55 per share in cash for their shares when Restaurant Brands International’s acquisition closed as of 11:59 p.m. on May 16, 2024. Carrols traded on Nasdaq under the ticker TAST. The parties estimate the class holds about 41.5 million shares.

Excluded are the defendants; anyone who was a director or named executive officer of Carrols or Restaurant Brands International at the closing; Garnett Station Partners and Cambridge Franchise Holdings; the spouses and children of those people; entities they control; and trusts or accounts that held Carrols shares for their benefit. Someone who sold before the closing is not in the class. A buyer whose purchase had not settled by the closing is treated as the holder of those shares, and the seller is not.

Why You Cannot Opt Out of This One

The Court preliminarily certified the class as a non-opt-out class under Delaware Court of Chancery Rules 23(a), 23(b)(1) and 23(b)(2). Those rules do not give class members a right to exclude themselves, so if the settlement is approved its release binds every class member and bars separate claims over the merger. A class member who disagrees can object to the settlement, the plan of allocation or the fee request instead. An objection must be received by November 9, 2026 and must include brokerage statements or a broker’s statement showing the objector held Carrols shares at the closing.

How Much Can You Get?

The settlement fund is $18.2 million plus interest. Dividing it by the roughly 41.5 million class shares gives about $0.44 a share before any deductions. Plaintiffs’ counsel will ask for attorneys’ fees and expenses of up to 20% of the fund, or $3.64 million. Incentive awards of up to $10,000 for each plaintiff would be paid out of that fee award, not on top of it. If the full 20% request is granted, the payout falls to about $0.35 a share before administration costs and taxes.

Cronos Group $10 Million Securities Class Action Settlement

September 28, 2026 12:00 PM

Investors who purchased or otherwise acquired publicly traded common stock of Cronos Group Inc. on the Nasdaq or another public U.S. market between May 9, 2019, and March 30, 2020, may be eligible to claim a cash payment from a class action settlement .

Cronos Group agreed to pay $10 million to settle a securities class action lawsuit alleging it and certain executives made false and misleading statements about its revenue. The lawsuit claimed Cronos improperly recognized revenue from three round-trip transactions, leading to restatements that reduced its revenue for the first and third quarters of 2019.

Who can file a claim?

The settlement includes all persons and entities, regardless of country of residence, who purchased or otherwise acquired publicly traded Cronos Group Inc. common stock on the Nasdaq or any other public U.S. market between May 9, 2019, and March 30, 2020, inclusive, and suffered damages as a result.

Additional details

  • Both individuals and entities can be class members.
  • The settlement includes investors who held shares directly and those who held through a broker or other nominee.
  • The actual beneficial owner or a legal representative must submit the claim.
  • Joint owners must each sign the claim form.
  • Executors, administrators, guardians, conservators and trustees may submit claims on behalf of others but must provide documentation proving their authority.
  • Claimants must submit a separate claim form for each account.

How much can class members get?

The total settlement fund is $10,000,000. The amount each class member will receive depends on several factors:

  • The number and value of valid claims submitted
  • The number of shares purchased or acquired during the class period
  • The timing of each purchase, sale and holding
  • The total recognized claims of all claimants

The settlement administrator will distribute payments on a pro rata basis according to the court-approved plan of allocation:

  • The estimated average recovery per eligible share is approximately $0.06 before deductions for attorneys' fees, litigation expenses, taxes and administration costs. The estimated average recovery after these deductions is approximately $0.04 per share.
  • Actual payments may be higher or lower depending on individual claims and the total number of valid claims.
  • The settlement administrator will calculate each class member's payment based on the recognized loss amount assigned to each share.
  • Recognized loss amounts depend on purchase and sale timing. The settlement administrator will calculate them using the court-approved plan of allocation.
  • For shares sold before Feb. 24, 2020, the recognized loss is $0.
  • For shares sold from Feb. 24, 2020, through March 30, 2020, the recognized loss is the lesser of the artificial inflation at purchase minus the artificial inflation at sale ( Table 1 on Page 17 of the settlement notice ) or the purchase price minus the sale price.
  • For shares sold from March 31, 2020, through June 26, 2020, the recognized loss is the lesser of the artificial inflation at purchase, the purchase price minus the average closing price through the sale date ( Table 2 on Page 18 of the settlement notice ) or the purchase price minus the sale price.
  • For shares held as of the close of trading on June 26, 2020, the recognized loss is the lesser of the artificial inflation at purchase or the purchase price minus $6.11 (the average closing price from March 31, 2020, through June 26, 2020).
  • The settlement administrator will set any recognized loss that calculates to a negative number to $0.
  • If total recognized claims exceed the fund, the settlement administrator will reduce payments on a pro rata basis.
  • Class members whose payment would be less than $10 will not receive a payout.

Generic Drug Price-Fixing End-Payer Settlements ($533M) — Sandoz, Sun/Taro, Apotex/Heritage
Proof or notice ID needed
Generic drug price-fixing settlement: claim from $533M paid by Sandoz, Sun/Taro and Apotex/Heritage by Nov. 9, 2026. Who qualifies and what proof you need.
Do I qualify?

Current Status

Claims are open. The deadline to file an end-payer claim, online or by mail, is November 9, 2026. All three settlements in this claim have final approval: Sandoz/Fougera ($275 million, approved September 26, 2025), Sun/Taro ($200 million, approved January 23, 2026) and Apotex/Heritage ($58 million, approved in late 2025). One claim covers all three. No payment date had been announced as of October 1, 2026.

This page covers the end-payer class action only. The state attorneys general run a separate consumer program with its own website and a later deadline of March 8, 2027; it is explained on its own page, the state attorneys general consumer settlement. Filing here does not file you there, and the November 9 deadline comes first.

What Changed Recently?

Class counsel combined the three finally approved end-payer settlements into one claims process, which opened in 2026 with a November 9, 2026 deadline. In September 2026 the state attorneys general also opened their own consumer claims process, with a March 8, 2027 deadline. That second program is a separate claim with separate money, which is why two deadlines appear in coverage of these settlements.

Which Program Is This?

Three separate legal tracks grew out of the same alleged price-fixing, and each has its own website and claim:

End-payer class action (this page) · official end-payer settlement website

Consumers and health plans that paid for covered generics

$533M from Sandoz/Fougera, Sun/Taro, Apotex/Heritage

About $96.5M from Apotex, Heritage, Lannett, Bausch, Glenmark

Direct purchaser class action

Pharmacies, wholesalers and distributors only

A consumer who qualifies for both consumer programs can file in both, but each needs its own claim.

Who Qualifies?

The end-payer classes cover consumers who paid all or part of the cost of covered generic medicines in the United States between May 1, 2009 and December 31, 2019, whether out of pocket, through a copay or through coinsurance, plus health insurers, employer plans and union benefit funds that reimbursed those costs. Consumers and third-party payers each have their own claim path.

The covered products include hundreds of common generic medicines, including widely used generics for blood pressure, heart conditions, cholesterol, diabetes, thyroid conditions, asthma, infections and depression. Check the full list on the official settlement website or in our covered generics list.

How Much Can You Get?

The three end-payer settlements total $533 million:

Final approval September 26, 2025 · claims open

Final approval January 23, 2026 · claims open

Final approval late 2025 · claims open

Payments are pro rata. What each claimant receives depends on how much was paid for covered generics and on the total value of valid claims, after court-approved fees and costs. No per-person amount has been announced.

What Proof Is Required?

Proof is required. You need records showing the covered generics you paid for during the class period, such as pharmacy purchase histories, receipts, insurer explanation-of-benefits statements or account statements. Most pharmacies can print a purchase history covering past years. Gather the records before you start the claim.

What Is the Deadline?

Submit the end-payer claim online or postmarked by November 9, 2026. Dates can change by court order, so confirm on the official settlement website. The separate state attorneys general consumer program has a different deadline, March 8, 2027, and needs its own claim on its own website.

How Do You File?

File online at the official end-payer settlement website or by mail. Choose the consumer or third-party payer path, list the covered generics you paid for, and attach your records. You do not need to sign up with or pay a claims-recovery firm; class counsel told the court in November 2025 that some outside firms were sending misleading solicitations.

Generic Pharmaceuticals Antitrust Class Action Settlement
Proof or notice ID needed
You may be able to claim a piece of this settlement if you paid for certain prescription generic drugs between May 1, 2009 and December 31, 2019.
Do I qualify?

Sun Pharmaceutical Industries Inc. and Taro Pharmaceuticals U.S.A. Inc. have agreed to a $200 million class action settlement to resolve claims they participated in an alleged scheme involving the pricing of certain generic drugs.

The generic medication settlement benefits consumers and third-party payers who indirectly purchased, paid for or provided reimbursement for certain generic prescription drugs between May 1, 2009, and Dec. 31, 2019. Eligible consumers generally purchased the drugs through pharmacies or mail-order pharmacies, while third-party payers may include insurers and self-funded employer health plans.

This settlement is part of the multidistrict litigation In re Generic Pharmaceuticals Pricing Antitrust Litigation , MDL No. 2724, pending in Pennsylvania federal court. The MDL includes antitrust claims involving multiple generic drug manufacturers and different groups of purchasers.

The Sun Pharmaceutical/Taro Pharmaceuticals agreement is separate from other settlements in the MDL, including earlier agreements involving direct-purchaser claims against Sun Pharmaceutical and Taro Pharmaceuticals and end-payer claims against Sandoz and Fougera . This settlement resolves end-payer claims against Sun Pharmaceutical and Taro Pharmaceuticals brought by consumers and third-party payers.

According to a class action lawsuit , generic drug manufacturers, including Sun Pharmaceutical and Taro Pharmaceuticals, engaged in an alleged scheme to fix, maintain and stabilize prices, rig bids and allocate markets and customers for certain generic drugs. Plaintiffs allege the conduct harmed competition and caused consumers and third-party payers to overpay for the drugs.

Sun Pharmaceutical and Taro Pharmaceuticals have not admitted any wrongdoing but agreed to a $200 million class action settlement to resolve the allegations.

Under the terms of the generic medication settlement, eligible class members who submit valid claims can receive a pro rata share of the net settlement fund. The proposed allocation plan provides that payments are generally based on the amount spent on the named generic drugs during the class period.

No specific payment estimate is available at this time. The final amount each claimant receives will depend on factors, including eligible purchases and the number of valid claims submitted.

The deadline for exclusion and objection was Dec. 2, 2025.

The final approval hearing for the generic medication settlement happened on Jan. 15, 2026.

To receive class action settlement benefits, class members must submit a valid claim form by Nov. 9, 2026.

The generic medication settlement benefits individuals and entities in the 50 United States, except Indiana and Ohio, as well as the District of Columbia, Puerto Rico and the U.S. Virgin Islands, that indirectly purchased, paid for or provided reimbursement for some or all of the purchase price of one or more named generic drugs, other than for resale, between May 1, 2009, and Dec. 31, 2019. A list of the named generic drugs can be downloaded from the notice section of the website.

Claimants can submit supporting documentation, such as pharmacy or medical records, receipts or insurance statements, along with their claim.

NOTE: If you do not qualify for this settlement do NOT file a claim.

Remember: you are submitting your claim under penalty of perjury . You are also harming other eligible Class Members by submitting a fraudulent claim. If you don’t qualify for this settlement, check out our database of other open class action settlements you may be eligible for.

Current Status

Claims are open. Claim forms for all three settlements must be submitted online or postmarked by November 9, 2026. The opt-out and objection deadline passed on October 1, 2026. The court will hold the final approval hearing on October 22, 2026 at 10:00 a.m. in Salt Lake City; it preliminarily approved all three settlements on June 22, 2026, and has not yet granted final approval.

What Changed Recently?

After rejecting an earlier version in March 2026 because the class had not been shown to meet the requirements for certification, the U.S. District Court for the District of Utah preliminarily approved three settlements on June 22, 2026 in Combs v. SafeMoon LLC, the investor class action over the SafeMoon (SFM) cryptocurrency token. Former SafeMoon CEO Braden John Karony, also a defendant, opposed preliminary approval; the court granted it over his objection. Notices were then issued, and the administrator, Stretto, opened claims through the settlement website.

The case was filed in February 2022 in California, against SafeMoon entities, company executives and several celebrity promoters, and was moved to Utah later that year. The investors allege that SFM tokens were sold as unregistered securities and promoted through misleading solicitations. Jake Paul and Daniel Keem deny all wrongdoing and liability and settled without admitting any. SafeMoon US filed for Chapter 7 bankruptcy in December 2023, so its settlement is with the bankruptcy trustee, Ellen E. Ostrow. Separately, a federal jury in Brooklyn convicted Karony in May 2025 of securities fraud conspiracy, wire fraud conspiracy and money laundering conspiracy in a criminal case brought in 2023.

Who Qualifies?

Each settlement has its own class, and a buyer may belong to more than one:

  • Trustee settlement: people and entities who, from March 8, 2021 through November 1, 2023, bought SFM tokens directly from SafeMoon US, LLC, or saw a solicitation for SFM tokens published by SafeMoon US and bought because of it.
  • Jake Paul settlement: people who in the same period bought SFM tokens directly from Jake Paul, or saw a solicitation for SFM tokens Paul published and bought because of it.
  • Daniel Keem settlement: people who in the same period bought SFM tokens directly from Daniel M. Keem, or saw a solicitation for SFM tokens Keem published and bought because of it.

The people named as defendants, including SafeMoon’s founders and former officers and directors, their immediate families and entities they control, are excluded.

How Much Can You Get?

None of the settlements pays a fixed amount. Each net fund is divided pro rata according to each claimant’s recognized loss:

  • Trustee settlement: an estimated $12 million, with a possible later payment if the trustee recovers other estate assets. Class counsel will ask for fees of up to 16.375% (about $1.965 million), up to $70,000 in expenses and up to $15,000 for the lead plaintiffs, leaving an estimated net fund of about $10 million. Payments under $50 are not made.
  • Jake Paul settlement: $200,000. With fees and expenses of up to 33% ($66,000), the net fund would be about $134,000. Payments under $10 are not made.
  • Daniel Keem settlement: $90,000. The notice estimates a net fund of about $61,000 after fees and expenses of up to 33%. Payments under $10 are not made.

A claimant’s recognized loss is what was paid for eligible SFM tokens bought from March 8, 2021 through November 1, 2023, minus anything received from selling them through December 14, 2023, matched first-in, first-out and converted to U.S. dollars at market prices on each date. Claimants with an overall market gain receive nothing, and a recognized loss cannot exceed a claimant’s actual market loss. Because the Paul and Keem funds are small compared with the trustee fund, their individual payments are likely to be modest.

Individuals who used Skin360 for a skin assessment in Illinois between Dec. 9, 2019, and May 5, 2023, may qualify to claim a cash payment from a settlement.
Do I qualify?

Johnson & Johnson agreed to a $4.7 million class action settlement to resolve claims that it violated Illinois privacy laws by collecting biometric data from its Skin360 app without consent.

The Neutrogena settlement benefits individuals who performed a Skin360 skin assessment using any version of the app, including Neutrogena Skin360, NeoStrata Skin360 or any Skin360 collaboration with another entity via mobile application or web application while in Illinois between Dec. 9, 2019, and May 5, 2023.

Plaintiffs in the class action lawsuit claim that Johnson & Johnson violated Illinois’ Biometric Information Privacy Act (BIPA) by collecting, storing, disclosing and profiting from biometric facial geometry from its Neutrogena skin app without first providing legally required written disclosures and obtaining written consent. BIPA applies to all biometric data collected in Illinois, including fingerprints, eye scans and facial geometry scans, such as those collected by the skincare app.

Johnson & Johnson is a consumer health company that sells skincare products under the name Neutrogena and other brands.

Johnson & Johnson has not admitted any wrongdoing but agreed to a $4.7 million class action settlement to resolve the BIPA allegations.

Under the terms of the Neutrogena settlement, class members can receive a cash payment. Exact payment amounts will vary depending on the number of claims filed with the settlement and the net settlement fund after deductions for fees and other expenses.

Johnson & Johnson also agreed to delete any images obtained during skin assessments and maintain a BIPA-compliant user notice and written consent mechanism going forward.

The deadline for exclusion and objection is Nov. 10, 2026.

The final approval hearing for the Neutrogena settlement is scheduled for Dec. 17, 2026.

Class members who wish to receive a settlement payment must submit a valid claim form by Nov. 25, 2026.

The class action settlement benefits Illinois residents who performed a Skin360 skin assessment using any version of Skin360, including Neutrogena Skin360, NeoStrata Skin360 or any Skin360 collaborations with other entities, via mobile or web application between Dec. 9, 2019, and May 5, 2023.

Biometric Privacy · Claims Open

Current Status

Claims are open. Claim forms are due November 10, 2026, online or by mail with a postmark by that date. The Circuit Court of Cook County has granted preliminary approval, and the final approval hearing is scheduled for November 23, 2026 at 9:30 a.m. No payment date has been set; payments come only if the court grants final approval.

What Changed Recently?

The court preliminarily approved a $635,160 settlement in Roman v. ZK Technology LLC, No. 2022-CH-06727, a biometric privacy class action before Judge Joel Chupack in the Circuit Court of Cook County, and notices went out to identified workers with the credentials needed to file online. The same settlement resolves a companion federal case with the same caption in the U.S. District Court for the Northern District of Illinois, No. 23-cv-15058.

ZK Technology LLC supplied finger-scan and face-scan timeclocks used by Alliance Ground International, LLC, an aviation ground-services company. The lawsuit alleges ZK Technology violated the Illinois Biometric Information Privacy Act (BIPA) by collecting or receiving AGI workers' fingerprint and face-geometry data for timekeeping without first giving the written disclosures and obtaining the written consent the law requires. ZK Technology denies violating BIPA, the court has not decided who is right, and the settlement is not an admission of wrongdoing.

Who Qualifies?

The settlement class covers people who, while working for AGI in Illinois at any time between September 2020 and August 12, 2026, scanned a finger or face on AGI's finger-scan or face-scan timekeeping system without consent having been obtained to share their biometric information with ZK Technology.

Excluded are anyone who opts out, anyone who signed a release of BIPA claims, anyone who was a member of a collective bargaining agreement, anyone who already released these claims in a related case dismissed before the settlement took effect, and the judge, court staff and their immediate families.

How Much Can You Get?

Every valid claimant receives an equal share of the $635,160 fund, capped at $2,000 per person. The notice estimates each payment at $800 to $2,000, depending mostly on how many valid claims are filed.

Several costs come out of the fund before it is divided: Class Counsel's attorneys' fees of up to 38% of the fund plus costs, a service award of up to $5,000 for the class representative, and settlement administration expenses, all subject to court approval.

On the claim form you choose how to be paid: a paper check or an electronic payment such as PayPal, Venmo or Zelle. Checks expire 120 days after they are issued.

What Proof or Notice ID Is Required?

No receipts or employment records are required, but filing online requires the Login ID and PIN printed on the notice mailed to you. The settlement website's online claim form will not open without both.

If you did not receive a notice or cannot find it, the settlement website offers a downloadable paper claim form that can be completed and mailed instead, or you can use the website's contact page to ask the administrator to resend your Login ID and PIN.

What Is the Deadline?

Online claims must be submitted by November 10, 2026. Mailed claim forms must be postmarked no later than November 10, 2026. The notice does not specify a time zone.

Requests to opt out and written objections must be postmarked by October 23, 2026, which is earlier than the claim deadline.

How Do You Take Action?

Go to the official ZK BIPA Settlement website, run by the court-appointed administrator Simpluris, and open the claim form. Enter the Login ID and PIN from your notice, confirm your information, pick check or electronic payment, and submit by November 10, 2026.

Current Status

Claims are open. The deadline to submit a claim online or by mail is November 10, 2026. The deadlines to opt out or object are earlier, and the official sources do not agree on them: the court-approved long-form notice states October 26, 2026, while the settlement website's FAQ page states November 10, 2026 — so anyone who wants to opt out or object should act by the earlier date. A final approval hearing is scheduled for December 1, 2026 at 10:00 a.m. ET before Judge J.P. Boulee in Atlanta. No final approval order has been entered and no payment date has been announced. Payments are made only if the court approves the settlement.

What Changed Recently?

The case is Doe v. Wellstar Health System, Inc., No. 1:24-cv-01748, in the U.S. District Court for the Northern District of Georgia, before Judge J.P. Boulee. Plaintiffs allege that Wellstar disclosed class members' personally identifiable information and protected health information to third parties — including Meta Platforms, Inc. and Google LLC — through tracking pixels, cookies and other tracking technologies installed on Wellstar's website and patient portal, and that deploying and using those technologies invaded class members' privacy.

Wellstar denies the allegations and denies doing anything wrong. The court has not decided who is right; the parties agreed to settle instead. A settlement is a resolution of disputed claims, not a finding of liability.

What has changed for readers is that notice has now gone out and the claim window is live, with a November 10, 2026 filing deadline and a December 1, 2026 final approval hearing on the calendar.

This is one of a series of settlements involving health systems and third-party web trackers. If you have been notified about more than one, our pages on the Penn Medicine, Allina Health and Duke MyChart settlements cover those separately — each has its own class, deadline and claim process.

Who Qualifies?

You are a member of the settlement class if you are a person residing in the United States whose information was disclosed to a third party between February 19, 2020 and July 22, 2026 through tracking technologies on Wellstar's website and patient portal.

Two things about that definition are worth drawing out. It is nationwide rather than limited to Georgia, where Wellstar operates — the test is where you reside, not where you were treated. And it does not, on its face, require that you were ever a Wellstar patient; it turns on whether your information was disclosed through the trackers on those web properties. In practice, most class members learn they qualify because a notice arrived, since the administrator works from the records identifying whose information was disclosed.

Excluded from the class are: any judge presiding over the action, that judge's staff, and immediate family members; officers and directors of Wellstar, along with its agents, affiliates, subsidiaries, parent companies, successors and predecessors, and any entity in which Wellstar or its parents hold a controlling interest; anyone who timely and validly opts out; the legal representatives, successors or assigns of any excluded person; and class counsel.

How Much Can You Get?

Wellstar agreed to pay $4,250,000 into a settlement fund. Class members who submit a valid claim share that fund on a pro rata basis — an equal share each — after the court-approved deductions come out.

No per-person figure has been announced, and there is no way to publish an honest one yet: a pro rata payment depends on how many valid claims are filed, which is not known until the claim window closes. Treat any specific dollar figure you see quoted elsewhere for this settlement as a guess.

City and County of San Francisco $5.75M Privacy Settlement

September 18, 2026 10:00 AM

Individuals who lived at Laguna Honda Hospital between April 14, 2022, and Aug. 16, 2023, or who resided there after March 2017 and received a written privacy violation notice may be eligible to receive a cash payment from a class action settlement .

The City and County of San Francisco agreed to pay $5.75 million to settle a class action lawsuit alleging Laguna Honda Hospital engaged in a pattern and practice of not complying with nursing home regulations meant to protect patients’ rights. The lawsuit claims those practices harmed patients and contributed to the decertification of the hospital and termination of its Medicare and Medicaid provider agreements. It also claims the hospital committed other violations of certain patients’ rights to privacy and confidentiality when handling their personal and medical information.

Who are the class members?

There are two classes. Individuals may be a member of one or both classes:

  • Patients’ rights class: All patients at Laguna Honda Hospital between April 14, 2022, and Aug. 16, 2023
  • Confidentiality class: All patients at LHH from March 23, 2017, and Feb. 17, 2026, or their responsible party, who received written notice from the San Francisco Department of Public Health that the hospital improperly shared their private medical and health information with unauthorized parties

How much can class members get?

The amount eligible class members can receive from the settlement depends on which class they belong to and, for some, how long they lived at Laguna Honda Hospital during the relevant period. Individuals who are a member of both classes will receive both payments.

  • Confidentiality class: Each member will receive a flat payment of $750.
  • Patients’ rights class: Each member will receive a base payment of $1,000. These class members will also receive an additional amount based on the number of days they resided at Laguna Honda Hospital between April 14, 2022, and Aug. 16, 2023.

Required documentation

Class members using the online distribution election form must provide the CPT ID and passcode they received in the class notice.

Payout options

Class members have two options for receiving their payment:

  • Option 1: They can receive a direct cash payment. Class members are responsible for managing these funds, including any impact on Supplemental Security Income or Medicaid eligibility. They must report any lump-sum payment to the Social Security Administration and their local Medi-Cal office by the tenth day of the month after they receive it. SSI asset limits are $2,000 a month for individuals and $3,000 a month for couples.
  • Option 2: They can receive the award in a pooled special needs trust, which can help preserve eligibility for SSI or Medicaid. CPT Institute manages the trust and can use the funds for a wide range of qualified expenses, such as medical care, support services and personal items. The manager generally pays funds directly to vendors and not to class members.

PPL $162M New York CDPAP Caregiver Wage Settlement
Paid automatically
Home-care workers paid through PPL in NYC and nearby counties from March 2025 to April 2026 are paid automatically, about $680 each; no claim form.

Wage & Hour · Pending — Automatic Payment

What Is This Settlement About?

Public Partnerships LLC (PPL), the company New York chose as its single statewide fiscal intermediary for the Consumer Directed Personal Assistance Program (CDPAP), has agreed to a $162 million settlement to resolve claims that it failed to pay home-care workers correctly during the program's 2025 transition. Plaintiffs allege that as CDPAP consolidated its payroll under PPL, personal assistants were paid late, paid incorrectly, or not paid at all, and that required wage-parity benefits were not provided as the law requires. The settlement is reported to be among the largest wage-and-hour class action settlements in New York history.

PPL has not admitted wrongdoing; the settlement resolves the claims without any court finding of liability. The case is Calderon v. Public Partnerships, LLC, in the U.S. District Court for the Eastern District of New York. Best of all for class members: this is an automatic-payment settlement, so eligible personal assistants do not have to file anything to be paid.

Who Qualifies?

The class is made up of roughly 200,000 current and former personal assistants — the home-care workers who provide hands-on care under CDPAP — who were paid through PPL for services performed in New York City, Nassau County, Suffolk County, or Westchester County between March 1, 2025 and April 30, 2026. These four downstate regions are collectively referred to in the settlement as the "downstate" area; the class is defined by that geography and time period, not statewide.

If you performed CDPAP work in those areas during that window and were paid (or were supposed to be paid) through PPL, you are likely a class member. Because payments are automatic, you do not need to sign up — but you should make sure PPL has your current contact information so your settlement notice reaches you.

How Much Can You Get?

Payments average about $680 per personal assistant — described as more than a full week's pay for many workers. Roughly 50,000 class members are expected to receive between $1,000 and $1,800. Individual amounts vary depending on how many hours you worked and how much you were underpaid during the class period. The $162 million fund also covers attorneys' fees, administration costs, and court-approved service awards, subject to the court's approval.

Do You Need to File a Claim?

No. This is a no-claim, automatic-payment settlement. If the court grants final approval, class members will be paid automatically — there is no claim form to submit and no documentation to provide. The settlement administrator, Atticus Administration, will send notices directly by email and text message, and notices will also be posted (in several languages) on the official settlement website. The only optional steps available to class members are to exclude themselves from (opt out of) the settlement or to object to it; neither is required to get paid.

Key Dates

• Preliminary Approval: July 1, 2026

• Class Notices: Sent by email and text after preliminary approval and posted to the official settlement website

• Opt-Out / Objection Deadline: To be set in the class notice — check the official settlement website

• Final Approval Hearing: November 10, 2026

• Payments: Issued automatically after final approval and any appeals are resolved

What the Lawsuit Alleged

When New York moved CDPAP to a single fiscal intermediary in 2025, PPL became responsible for onboarding and paying hundreds of thousands of personal assistants at once. Plaintiffs allege that during that transition PPL failed to pay personal assistants accurately, on time, or at all, and failed to provide benefits compliant with New York's Home Care Worker Wage Parity Act — allegedly violating state and federal wage-and-hour law. These are the plaintiffs' allegations; PPL denies wrongdoing, and the settlement resolves the claims without any admission of liability.

Anthem Grandfathered Rx Brand-Name Drug Settlement

Anthem will reimburse out-of-pocket costs for brand-name (single source) drugs denied under Core 5000, Tonik & RightPlan plans. Claim by Nov 10, 2026.

Anthem Blue Cross Prescription Drug Denials Settlement

Consumers covered under an Anthem Blue Cross Life and Health Insurance Co. health plan who paid out of pocket for a brand name (single-source) prescription drug between Jan. 1, 2017, and May 8, 2026, may be eligible to claim expense reimbursement from a class action settlement .

Anthem Blue Cross Life and Health Insurance Co. agreed to settle a class action lawsuit claiming it failed to cover medically necessary brand name prescription drugs for certain “grandfathered” health insurance policies as required by California law. The settlement also resolves claims that Anthem charged some members an $8,850 deductible for brand-name prescription drugs.

Who can file a claim?

Class members must meet all of the following criteria:

  • One of these Anthem health plans covered them at any time between Jan. 1, 2017, and May 8, 2026:
  • RightPlan (with generic prescription drug coverage)
  • They personally paid out of pocket for a brand name (single-source) prescription drug during this period.
  • Their health plan would have otherwise covered the prescription, but Anthem denied it solely because it was a brand name (single-source) product.
  • Any other health plan or third party did not reimburse them for their out-of-pocket costs for the drug.

Each individual seeking reimbursement must submit a separate claim form. Family members cannot combine their claims on a single form. Those who need additional space to list their expenses can download extra claim forms from the official settlement website or request them by emailing info@GrandfatheredRXSettlement.com .

How much can class members get?

Eligible class members can receive reimbursement for the full amount of what they paid for out-of-pocket expenses for brand name (single-source) prescription drugs that Anthem denied solely because they were brand name products.

Out-of-pocket expenses do not include any costs covered or reimbursed by another insurance plan, health care service plan or from the proceeds of any other judgment or settlement specifically designated for medical expenses.

For those who paid all or part of the $8,850 deductible for brand name prescription drugs, Anthem will automatically mail a refund for the amount they paid. Class members do not need to submit a claim form to receive this refund.

How to claim a settlement payment

Eligible class members can file a claim online or download and print a PDF claim form , complete it and mail or email it to the settlement administrator:

  • Settlement administrator's mailing address: Special Notice of Defense, c/o Rust Consulting Inc. - 9167, PO Box 2599, Faribault, MN 55021-9599
  • Settlement administrator's email address: info@GrandfatheredRXSettlement.com

What proof or documentation is necessary to submit a claim?

All class members must submit the following documentation with their claim form:

  • A copy of the prescription or written verification from the prescribing provider or dispensing pharmacy for each drug listed
  • Proof of payment showing the class member personally paid out of pocket for the drug (such as a pharmacy receipt, credit card statement or cancelled check)

To file an online claim , class members must provide the unique claimant ID and access code from the settlement notice they received.

The settlement administrator will not process claims that do not contain all required documentation. If a class member's claim is incomplete, the administrator will contact them with one opportunity to submit supplemental information. If they do not provide adequate proof, the administrator will deny the claim.

Payout options

The settlement administrator will automatically mail deductible refunds to the policyholder's address on file with Anthem. It will mail out-of-pocket cost reimbursement for approved claims to the address the class member provides on the claim form.

Securities · Pending — Automatic Payment

Current Status

There is no claim form and no claim deadline. Eligible class members are paid automatically if the Court approves the settlement, which it preliminarily approved on September 17, 2026. The dates that matter are procedural: objections must be received by November 10, 2026, and the settlement hearing is scheduled for December 4, 2026 at 11:00 a.m. Eastern Time before Chancellor Kathaleen St. Jude McCormick in the Delaware Court of Chancery in Wilmington. No final approval has been granted and no payment date had been announced as of October 19, 2026.

What Changed Recently?

The parties signed the Stipulation and Agreement of Settlement on September 16, 2026, and on September 17, 2026 the Court preliminarily approved the settlement, authorized notice to the class and scheduled the December 4 hearing. The long-form notice is dated October 5, 2026, and Kroll Settlement Administration published the summary notice on October 19, 2026. The settlement resolves City of Sarasota Firefighters’ Pension Fund v. Dunleavy, C.A. No. 2022-0698-KSJM, for $44 million in cash.

The settlement amount is paid by Inovalon and its insurers; under the stipulation, none of the defendants contributes to it. It is an all-in figure, so attorneys’ fees and notice and administration costs come out of the $44 million. The agreement followed a September 2025 mediation that ended without a deal; in July 2026 the mediator proposed settling for $44 million, and both sides accepted. The defendants deny all allegations of wrongdoing, fault, liability or damage and say they settled only to avoid the burden and expense of further litigation.

Who Qualifies?

The class covers all record and beneficial holders of Inovalon Holdings, Inc. Class A common stock as of November 24, 2021, the day the take-private closed, who received $41.00 in cash for each share, along with their successors and assigns. Inovalon traded on Nasdaq under the ticker INOV until the deal closed. The class covers Class A stock only.

Excluded are the defendants and former defendants, which include Inovalon itself, founder Keith R. Dunleavy and his related entities, early investor André Hoffmann and his investment vehicle, and the former directors named in the case; their parents, subsidiaries, affiliates, partners, executive officers, directors and controlling persons; their immediate families; and any entity in which any of them had a controlling interest on November 24, 2021. A buyer whose purchase had not settled before the closing is treated as the holder of those shares, and a seller of those shares is not.

Why There Is No Opt-Out

On August 18, 2025, the Court certified the class as a non-opt-out class under Delaware Court of Chancery Rules 23(a), 23(b)(1) and 23(b)(2), so class members cannot exclude themselves, and if the settlement is approved its release binds every class member. A class member who disagrees can object to the settlement, the plan of allocation or the fee and expense application instead.

An objection must be filed with the Court and served on plaintiffs’ and defendants’ counsel so that it is received by November 10, 2026. It must name the case, give the objector’s contact information and signature, explain the specific reasons for the objection, and include documents proving class membership. An objector who wants to speak at the hearing includes that request in the written objection, and an attorney representing an objector must file a notice of appearance so that it is received by November 18, 2026.

How Much Can You Get?

The settlement fund is $44 million plus interest. Each eligible class member’s payment equals the number of eligible Class A shares held at the closing multiplied by a per-share recovery: the net settlement fund divided by the total eligible shares held by all eligible class members. The notice does not state an estimated per-share amount.

Data Breach · Claims Open

Current Status

Claims are open. Claim forms must be submitted online or by email, or postmarked if mailed, by November 12, 2026. The deadline to exclude yourself or object is earlier — October 13, 2026 — and that is the date that decides whether you are bound by the release. The court has scheduled a final approval hearing for December 7, 2026 at 9:00 a.m. Central Time, to be held by Zoom, and the settlement has not yet been approved. No payment date has been announced: the notice states that payments will be distributed only if the court grants final approval and after any appeals are resolved, and that it is not known whether appeals will be filed.

What Changed Recently?

The settlement is newly open to claims. Susan B. Allen Memorial Hospital has agreed to resolve In Re: Susan B. Allen Data Security Litigation, Case No. BU-2025-CV-000185, in the District Court of Butler County, Kansas, and the court has authorized notice to the class. Simpluris is administering the settlement.

The hospital denies that it did anything wrong, and the court has not decided who is right. Both sides agreed to settle to avoid the costs, risks, disruption and uncertainty of continued litigation. No finding of liability has been made against the hospital.

What the Case Is About

The lawsuit concerns a targeted cyberattack on the hospital's computer systems in July 2025. According to the notice, certain files containing private information were accessed during the incident, and those files may have contained names, dates of birth, health insurance information, billing information and medical treatment information.

That combination is what makes this a medical-identity exposure rather than an ordinary payment-card breach, and it is reflected in the monitoring product the settlement provides, which watches for misuse of insurance IDs and medical record numbers rather than just credit activity.

Who Qualifies?

The court defined the class simply: all persons who were sent written notification of the data incident.

That is an unusually clean definition — eligibility turns on whether the hospital sent you a notice, not on when you were a patient or what was exposed in your particular file. The notice also says the hospital's records indicate that recipients are class members entitled to benefits, and that some people may have received an earlier notice directly from the hospital before this court-authorized one.

Only two groups are excluded: the judge in the case along with the judge's family and staff, and anyone who perpetrated the data incident.

How Much Can You Get?

Every class member can claim the monitoring, and separately may claim one or both cash options — subject to a cap that is easy to miss.

  • Two years of CyEx Medical Shield Complete. Available to every class member. What is this Lawsuit about?

A Settlement has been reached with Autobell Car Wash, LLC, (“Autobell” or “Defendant”) in a class action lawsuit. This class action lawsuit concerned the targeted cyberattack on Autobell’s computer systems that occurred between April 1, 2024 and April 7, 2024 (the “Data Incident”). Certain files that contained private information were accessed. These files may have contained personal information such as names and Social Security numbers.

Who’s Included?

The Court has defined the Settlement Class as: “all natural persons residing in the United States who were provided notice that their personal information was involved in the Data Incident.”

What does the Settlement Provide?

You may claim Credit Monitoring Services, as defined below. Additionally, you may submit a claim for either Documented Unreimbursed Losses and Time Spent or an Alternative Cash Payment.

CREDIT MONITORING SERVICES. All Settlement Class Members are eligible to receive 2 years of Credit Monitoring Services. This includes:

  • real time monitoring of the Settlement Class Member’s credit file at three bureaus;
  • dark web scanning with immediate notification of potential misuse;
  • comprehensive public record monitoring;
  • medical identity monitoring;
  • identity theft insurance with no deductible; and
  • access to fraud resolution agents to help investigate and resolves instances of theft

CASH PAYMENTS. All Settlement Class Members are eligible for a cash payment, subject to the terms below.

Documented Unreimbursed Losses. If you incurred actual, documented out-of-pocket losses due to the Data Incident, you may file a claim for reimbursement. The maximum amount of this reimbursement is $5,000.00.

This benefit covers expenses like:

  • bank, credit card, and debit card fees;
  • overdraft, declined payment, and returned check fees;
  • cost to replace your driver's license, Social Security number, or other types of identification;
  • long distance phone charges, cell phone charges (if charged by the minute), or data charges (if charged by the amount of data used);
  • fees for credit reports or credit monitoring; and
  • losses due to identity theft or fraud

You must submit third-party documentation, such as receipts, to verify the costs you incurred. You may submit “self-prepared” documents to clarify or support other submitted documentation, but self-prepared documents by themselves are not sufficient to file a valid claim.

Time Spent. In addition to your claim for Documented Unreimbursed Losses, you may submit a claim for reimbursement for time you spent dealing with the Data Incident, up to four (4) hours at $25.00 per hour.

To receive reimbursement for Time Spent, you must submit a sworn statement, attested by signature under penalty of perjury, stating how the time was spent, demonstrating that it was related to the Data Incident. Any reimbursement for Time Spent is included in calculating the $5,000.00 maximum payment under Documented Unreimbursed Losses.

If you claim Documented Unreimbursed Losses and/or Time Spent, you may not also claim an Alternative Cash Payment.

Alternative Cash Payment. In lieu of submitting a claim for Documented Unreimbursed Losses and/or Time Spent, Settlement Class Members may choose to instead receive a one-time cash payment of $45.00.

If you wish to make a claim for an Alternative Cash Payment, you may not make a claim for Documented Unreimbursed Losses and Time Spent.

San Diego $40M Water Rate Settlement — Automatic Refunds
Paid automatically
San Diego will pay $40M over single-family water rates charged from 2014 to 2023. Overcharged customers are refunded automatically. Opt out by Nov. 12, 2026.
Do I qualify?

Water Rate Refunds · Pending — Automatic Payment

Current Status

There is no claim form. Single-family residential customers who were overcharged under the court-approved refund calculation will be paid automatically if the settlement is approved. The deadline to opt out or object is November 12, 2026. The final approval hearing is scheduled for December 11, 2026 at 9:00 a.m. PT; no final approval order or payment date had been announced as of October 2, 2026. Class members can optionally confirm their contact details and choose how they want to be paid on the settlement website.

What Changed Recently?

The City of San Diego agreed to pay $40 million to settle Patz v. City of San Diego, a class action over the water rates the City charged single-family residential customers. The settlement agreement was signed in July 2026, after the parties accepted a mediator’s proposal from a retired San Diego Superior Court judge, and notice is now going out to class members.

The case was filed in July 2015. The plaintiffs claim the City’s single-family residential water rates were unconstitutional because they were not proportional to the cost of providing water service to those customers. The court certified the class in August 2019 and, after a bifurcated trial, entered judgment for the class on March 30, 2022. The City appealed, and the plaintiffs cross-appealed over the size of the refund. The City put new single-family residential water rates in place on December 1, 2023. On July 30, 2025, the Court of Appeal affirmed the judgment in a published opinion, with directions, and sent the case back to the trial court.

The City denies that it overcharged any class member or that any refund is owed, and the settlement is not an admission that it owes money. According to the notice, the court had not yet decided how a newer state law, Government Code section 53758.5 (added by Senate Bill 1072), applies to the refund award. The settlement resolves the case without that question being decided. No court has ruled on whether the City’s current water rates are constitutional.

Who Qualifies?

The settlement class is every single-family residential customer of the City of San Diego who received water service at any time between August 14, 2014 and December 1, 2023. The settlement agreement estimates the class at about 436,069 people. That includes customers who moved into the City’s service area after the original class notice went out in March 2020.

Excluded are City of San Diego officers and directors, class counsel, the assigned judge and that judge’s immediate family and judicial staff, and the 1,068 people who opted out of the class after it was certified, by the May 30, 2020 deadline.

Being in the class does not guarantee a payment. Only class members who were overcharged under the court-approved refund calculation receive money. The notice says some customers were not overcharged under that calculation, and they will not receive a refund.

How Much Can You Get?

No per-customer amount has been published. Each refund is a share of the net settlement fund, which is the $40 million minus court-approved costs. Those costs can include:

  • Attorneys’ fees of up to one-third of the fund ($13,333,333.33)
  • Litigation expenses of up to $500,000
  • Service awards of up to $7,500 each for the two class representatives
  • Settlement administration costs, expected to be no more than $199,000

Hunter Warfield $350 Unlicensed Rental Collections Settlement (Maryland)
Paid automatically
Maryland tenants pursued by Hunter Warfield for rent on unlicensed rental properties are paid $350 automatically under the settlement; no claim form to file.
Do I qualify?

Debt Collection · Pending — Automatic Payment

What Is This Settlement About?

Debt collector Hunter Warfield, Inc. agreed to a class action settlement to resolve claims that it pursued Maryland tenants for rent tied to rental properties that lacked the license Maryland law requires. The case — Andrew Blizzard et al. v. Hunter Warfield, Inc., in the U.S. District Court for the District of Maryland — alleged the company engaged in debt collection and related activity against tenants over rent they were said to owe during periods when the underlying rental property was unlicensed. Hunter Warfield denies wrongdoing and agreed to the settlement to resolve the matter.

The settlement is structured as an automatic payment: eligible households do not need to file anything. If the court grants final approval, the settlement administrator will mail $350 to each qualifying household on the court-approved class list, drawn from Hunter Warfield's records.

Who Qualifies?

The settlement class is made up of Maryland tenants who meet all of the following: they lived at a residential rental property in Maryland; the property lacked the rental license required under Maryland law during the relevant period; and Hunter Warfield sued them in a failure-to-pay-rent case, or sent them to collections, for rent they allegedly owed during that unlicensed period. Class membership is determined from Hunter Warfield's records and the court-approved class list rather than from anything you file.

For payment purposes, a "household" is each unique leased unit shown in those records. If more than one person was on a lease, the $350 is tied to the unit, not to each individual tenant.

How Much Can You Get?

Each eligible household receives $350. Because this is an automatic-payment settlement, there is no benefit tier to choose, no receipts to submit, and no online claim to file. The administrator issues payments to the households on the class list after the settlement becomes final. Checks must be cashed within 120 days of issuance; funds from checks that are never cashed are redirected to a nonprofit rather than returned to the defendant.

What You Need to Do

For most class members, the answer is nothing — payment arrives automatically if the settlement is approved. The one thing worth doing is making sure the administrator has your current mailing address. If you have moved since you received your settlement notice, update your address through the official settlement website so your check reaches you. The website's contact page can also help if you received a notice but have questions about your household's status.

Class members who want to keep the right to sue Hunter Warfield individually must opt out by the deadline stated in their notice; those who do nothing remain in the class, receive the automatic payment, and are bound by the settlement's release. The final approval hearing is set for August 25, 2026.

Current Status

Payments are automatic and there is no claim form. Vice Chancellor Bonnie W. David will consider final approval at a settlement hearing on December 7, 2026 at 1:30 p.m. Eastern Time. Written objections must be received by November 12, 2026. The class is a non-opt-out class, so no one can exclude themselves. No payment will be made until the Court approves the settlement and the approval becomes final.

What Changed Recently?

The parties to In re Thoughtworks Holding, Inc., Consolidated C.A. No. 2024-1295-BWD, signed a Stipulation and Agreement of Settlement, Compromise, and Release on August 31, 2026, and Vice Chancellor Bonnie W. David entered a Scheduling Order on September 15, 2026 directing notice to the class and setting the settlement hearing. The court-authorized notice, dated October 8, 2026, has been mailed or emailed to registered holders and posted on the official settlement website run by JND Legal Administration.

The parties first agreed to resolve the case for $31 million on June 17, 2026, after an April 2026 mediation that did not produce a deal on the day and months of further talks. A five-day trial had been scheduled for the week of December 7, 2026, the same week the settlement hearing is now set.

What Was the Lawsuit About?

On August 5, 2024, Thoughtworks, a global technology consultancy, announced that funds advised by Apax Partners LLP would buy every share they did not already own for $4.40 in cash. The same day, Apax affiliate Turing EquityCo II L.P., which held a majority of the stock, approved the merger by written consent, so the deal did not go to a vote of minority stockholders. It closed on November 13, 2024. The company had gone public in September 2021 at $21 a share, according to the complaint.

The class action complaint, filed April 2, 2025, alleges that Apax, as controlling stockholder, breached its fiduciary duties by taking the company private at an unfair price through an unfair process, and that five Thoughtworks directors, including CEO Michael Sutcliff, breached theirs by approving the deal. It claims the special committee and its financial adviser were conflicted, that there was no market check and no minority vote, and that the price was well below the $12 a share Apax agreed to in June 2023 before withdrawing that proposal in July 2023. It also claims the price failed to credit the value expected from the newly appointed CEO and sat at the low end of, or below, the valuation ranges in the special committee adviser’s own analyses.

Defendants deny all of the allegations and any wrongdoing or liability. They argue that an independent special committee of four directors negotiated and recommended the deal, and that $4.40 was a fair price that carried a substantial premium to the unaffected stock price. The settlement is not an admission by any defendant, and the Court has made no findings on the merits. Several Thoughtworks holders also filed appraisal petitions in the same court, and those cases were consolidated with the class action in April 2025.

Who Qualifies?

The Court certified the class on May 1, 2026. It covers all record holders and beneficial owners of Thoughtworks common stock whose shares were exchanged for, or had the right to receive, $4.40 per share in cash at the November 13, 2024 closing, along with their heirs, successors, transferees and assigns. The parties estimate the class holds about 119 million shares.

Excluded are the defendants; anyone who was a Thoughtworks director or officer immediately before the closing and their immediate families; entities any of them control; their parents, subsidiaries and general partners; and trusts, estates or accounts that held shares for their benefit. A buyer whose purchase had not settled before the closing is treated as the class member for those shares, and the seller is not.

California Bail Bond Price-Fixing Settlements (~$69M)
Check requirements
People who paid a California bail bond premium may be covered by about $69 million in settlements over alleged price-fixing by insurers; no claim form yet.
Do I qualify?

Antitrust · Pending Approval

Current Status

There is nothing to file yet, and no deadline to worry about. The largest settlements in this case — reported at roughly $66.3 million from nine surety insurers — were filed with the U.S. District Court for the Northern District of California in June 2026 and are awaiting preliminary approval. Only after the court grants preliminary approval will a notice program and claim process be set, with deadlines announced through the official settlement website. Earlier, smaller settlements in the same case bring the combined total to approximately $69 million. No final approval has been granted for the new settlements and no payment date exists. The practical step for now: if you paid a California bail bond premium, keep any paperwork you have and watch the official settlement website.

What Is This Case About?

The case is In re California Bail Bond Antitrust Litigation, Case No. 4:19-cv-00717-JST, pending in the U.S. District Court for the Northern District of California in Oakland since 2019. The lawsuit alleges that bail bond surety insurers — the companies that underwrite the bonds bail agencies sell — conspired for roughly two decades to keep California bail bond premiums artificially high, typically around 10% of the bail amount, rather than competing on price. The complaint asserts claims under the Sherman Act, California's Cartwright Act, and California's Unfair Competition Law against what was originally a group of 21 companies.

The defendants deny the allegations, and no court has found that any of them violated the law. As with any settlement, the insurers that have agreed to pay are resolving claims without admitting wrongdoing.

Beyond the money, reporting on the proposed settlements describes conduct changes: participating insurers must refrain from coordinating bail bond pricing with competitors and must notify licensed bail bond agents that premiums are negotiable rather than fixed.

Who Qualifies?

The settlement class generally covers people who paid part or all of the premium on a commercial bail bond posted in connection with a California state criminal proceeding, with a class period reaching back to February 24, 2004. That includes not just the person who was bailed out but the family members and friends who actually paid or co-signed the premium — which is often who bore the cost.

Reporting on the proposed settlements estimates roughly 2 million class members. The exact class definition and period for the newest settlements will be spelled out in the court-approved notice once preliminary approval is granted; earlier settlements in the case used a class period running from February 24, 2004 through April 25, 2024.

How Much Can You Get?

No per-person figure has been announced, and with a fund of roughly $69 million across approximately 2 million class members, individual payments will depend heavily on the court-approved plan of allocation — likely tied to the premiums each class member actually paid. Bail premiums are substantial sums: 10% of a $50,000 bail is $5,000, so allocations weighted by premium size could matter a great deal here. That is our observation about how these funds typically work, not an announced term; the actual allocation plan will come with the notice.

What Should You Do Now?

Nothing needs to be filed yet. Two things are worth doing in the meantime. First, gather whatever records you have of the bail bond premium you paid — the bail agency agreement, receipts, payment plan statements, or the name of the agency and rough dates. Whether the eventual claim process requires documentation is not yet known, but records can only help. Second, watch the official settlement website, CA Bail Bond Antitrust.com, which is maintained by the court-appointed administrator and will carry the notice, deadlines, and claim form when they exist.

September 14, 2026 4:01 PM

Participants or beneficiaries in the Ampam Parks Mechanical Inc. Employee Stock Ownership Plan with vested stock on or after Aug. 6, 2023, may be eligible to receive a payment from a class action settlement .

Ampam Parks Mechanical agreed to pay $13.75 million to settle a class action lawsuit alleging violations of federal law related to the administration and management of its ESOP. The plaintiff claimed the company and other defendants breached their fiduciary duties under the Employee Retirement Income Security Act during certain transactions involving the sale of Ampam stock to and from the ESOP, resulting in financial harm to plan participants and beneficiaries.

Who are the class members?

Class members include all participants and beneficiaries in the Ampam Parks Mechanical ESOP who held vested Ampam stock on or after Aug. 6, 2023.

How much is the settlement payment?

How to claim a class action payment

Eligible class members do not need to file a claim to receive a settlement payment.

Class members who prefer to receive their payment as a rollover to a qualified retirement account to avoid tax withholding can submit a rollover form online or print and complete the PDF rollover form and mail it the settlement administrator.

  • Fairness hearing : Dec. 3, 2026

When is the Ampam ESOP settlement payout date?

The settlement administrator will distribute payments to eligible class members after the court resolves any appeals and grants final approval of the settlement.

Why is there a class action settlement?

The class action lawsuit alleged Ampam Parks Mechanical and other defendants violated ERISA by causing the ESOP to buy Ampam stock at an inflated price in 2019 and sell it at a deflated price in 2023. The plaintiffs claimed these actions breached fiduciary duties and harmed ESOP participants.

Ampam Parks Mechanical denied any wrongdoing but agreed to settle to avoid the cost and risk of ongoing litigation.

Social Casino · Claims Open

Current Status

Claims are open. Claim Forms must be submitted by January 4, 2027, and that same date is also the deadline to exclude yourself from the class and the deadline to object — this settlement does not set the earlier opt-out cutoff most do. The final approval hearing is scheduled for February 8, 2027, so the court has not yet decided whether to approve the settlement, and no payment date had been announced as of October 1, 2026. If you bought coins in either app while in Washington during the class period, the action to take now is to file a Claim Form. You do not need receipts, and you do not strictly need the notice.

What Changed Recently?

The settlement schedule has been pushed back. The official settlement website now lists January 4, 2027 as the shared deadline to file a claim, opt out or object — replacing the November 13, 2026 date in the original notice — and February 8, 2027 for the final approval hearing, replacing December 14, 2026. The case is captioned Larsen v. PTT, LLC (d/b/a High 5 Games, LLC) and High 5 Entertainment, LLC, No. 3:18-cv-05275-TMC, before Judge Tiffany M. Cartwright in the U.S. District Court for the Western District of Washington.

What sets this settlement apart from nearly every other one on this site is how it got here. The notice says the parties agreed to settle following a jury trial in which the jury returned a verdict in the plaintiff's favor. Most class settlements are struck to avoid a trial; this one was struck after one, to avoid the further uncertainty and expense of post-verdict litigation and appeals, and to get money to class members sooner.

The lawsuit alleges that the defendants violated Washington State's gambling laws and its Consumer Protection Act through the operation of the High 5 Casino and High 5 Vegas social casino apps and the sale of virtual coins inside them. Washington law allows the recovery of money lost on gambling games, which is the theory that makes these cases work in that state and largely nowhere else. The defendants deny all claims and deny that they violated any law.

Who Qualifies?

The court defined the class as all individuals in Washington who played or purchased virtual coins on either High 5 Casino or High 5 Vegas after April 9, 2014 and before October 1, 2022. That includes people reasonably determined, by billing address information, IP address information or other information furnished by the platform providers, to be located in Washington or associated with purchases in Washington.

Two things follow from that. First, membership is not limited to people who typed a Washington address into an app store — an IP address or purchase data tying you to Washington can put you in the class. Second, the class covers people who merely played as well as those who paid, but only class members who actually purchased virtual coins can receive a cash payment.

Excluded are any judge or magistrate presiding over the action and members of their families; the defendants, their subsidiaries, parent companies, successors, predecessors and any entity in which a defendant or its parent has a controlling interest, along with their current or former officers, directors and employees; anyone who properly and timely opts out; and the legal representatives, successors or assigns of any excluded person. If you were emailed or mailed a notice of the settlement or of the earlier class certification order, the administrator's records already show you as a class member.

The final approval hearing is January 22, 2027, and no payment date has been set.

What Changed Recently?

Sandwich SC LLC, which does business as Jimmy John’s, agreed to pay $975,000 to settle a class action under Washington's pay-transparency law, RCW 49.58.110, part of the Equal Pay and Opportunities Act. That law requires employers to include a wage scale or salary range and a general description of benefits and other compensation in job postings. The case is Jeffrey Hill v. Sandwich SC LLC, No. 25-2-08587-2 SEA, in King County Superior Court. It was filed March 17, 2025, paused in May 2025 while the Washington Supreme Court decided a related question about who can sue under the law, and settled after limited discovery. The $975,000 is what remained of the company's $1 million employment practices liability insurance policy, and the deal was reached in response to a policy-limits demand. The court granted preliminary approval on August 24, 2026. Sandwich SC denies that it violated the law, and the court has not decided who is right.

Who Qualifies?

The settlement class is everyone who, from January 1, 2023 through August 24, 2026, applied for a job opening in Washington with Sandwich SC LLC, where the job posting did not disclose a wage scale or salary range and/or a general description of benefits and other compensation to be offered to a hired applicant. The company told the court there are about 1,508 class members. Class members are identified from the company's application records and are being notified by mail and email; someone who believes they were left out can raise it with the settlement administrator, Simpluris, within 30 days of the notice going out, and the company's records control unless the applicant has documents showing otherwise.

How Much Can You Get?

Every class member who does not opt out gets an equal share of the Class Fund: the $975,000 minus court-approved attorneys' fees and costs, the plaintiff's service award and administration costs. Class counsel, Emery Reddy, PC, is asking for $321,750 in fees and costs (about one-third), the plaintiff is asking for a $20,000 service award, and administration costs are capped at $40,000.

If the court approves those amounts and administration costs reach the cap, $593,250 would be left to split among about 1,508 people, or about $393 each. That is an OCA estimate; each class member's notice states their own estimated payment, and shares grow if the court awards less, administration costs come in lower or people opt out. The settlement agreement describes the payments as non-wage damages reported on a Form 1099. Money left after uncashed checks expire goes to Washington's unclaimed property program in the class member's name, and any remaining residual goes to the Legal Foundation of Washington.

What Proof or Notice ID Is Required?

None to be paid. There is no claim form. The Login ID and PIN printed on the notice are needed only to use the settlement website's form to update a mailing address or switch to a digital payment.

What Is the Deadline?

There is no claim deadline. Requests to be excluded must be mailed and postmarked by November 13, 2026. Objections must be filed with the court, with copies mailed to both sides' lawyers, by the same date. The address-update and payment-selection form on the settlement website also closes November 13, 2026.

Compucom Systems $1M Washington Job Applicant Settlement
Paid automatically Wage And Hours
Employment seekers who applied for a job at Compucom in Washington state between January 2023 and September 2026 may qualify to receive a settlement payment.
Do I qualify?

Employment · Pending — Automatic Payment

Current Status

The King County Superior Court preliminarily approved the CompuCom settlement on September 16, 2026, and notices have gone out from the settlement administrator. There is no claim form: class members are paid automatically if the court grants final approval at a hearing on January 15, 2027. The deadline to opt out or object is November 13, 2026. No payments have been made yet.

What Changed Recently?

The settlement is new. In Spencer v. CompuCom Systems, Inc., a job applicant claimed that CompuCom’s postings for open Washington positions did not list a wage scale or salary range and a general description of benefits, as Washington’s Equal Pay and Opportunities Act (RCW 49.58.110) requires. The complaint was filed in King County Superior Court in February 2025. The case was then paused while the Washington Supreme Court answered a certified question about the same law in Branson v. Washington Fine Wine & Spirits, and it resumed after that decision became final in September 2025.

CompuCom, an IT services company, agreed to pay $1,000,000 to resolve the case. It denies the claims, and the settlement is not an admission of wrongdoing. Judge Ken Schubert granted preliminary approval on September 16, 2026.

Who Qualifies?

The settlement class covers people who, from January 1, 2023 through September 16, 2026, applied for a CompuCom Systems job opening in Washington State where the job posting did not disclose a wage scale or salary range and a general description of benefits and other compensation.

Class membership comes from CompuCom’s own applicant records. People who received the mailed or emailed notice are, according to those records, members of the class. Anyone who opts out by November 13, 2026 is no longer in the class and receives nothing from the settlement.

How Much Can You Get?

The settlement website estimates each class member’s payment at $266.67. Every class member who stays in receives an equal share of the “class fund,” which is the $1,000,000 total minus the amounts the court approves for other uses:

  • Attorneys’ fees of up to $250,000 plus up to $5,000 in litigation costs, which class counsel says it will request
  • A $15,000 service award for the named plaintiff, also subject to court approval
  • Settlement administration costs, capped at $30,000 under the settlement agreement

The settlement agreement says CompuCom identified 2,626 class members. Any amount the court declines to award for fees, costs or the service award, and any money from people who opt out, goes back into the class fund and is shared among the remaining class members. The $266.67 figure is an estimate, and the final amount can change.

For context, the original complaint sought $5,000 in statutory damages for each class member under RCW 49.58.070. The settlement replaces those claims with the equal-share payment.

What Proof or Notice ID Is Required?

None, because there is nothing to file. This is an automatic-payment settlement: if the court grants final approval, each class member who did not opt out is mailed a check at the address the administrator has on file.

The settlement website has an optional address-update and payment-selection form. It requires the LoginID and PIN printed on each person’s notice, but it is used only to change a mailing address or switch from a paper check to an electronic payment. A class member who never uses the form is still paid.

What Is the Deadline?

There is no claim deadline. The dates that matter are:

  • November 13, 2026 — deadline to opt out, to object, and to submit an address update or electronic payment selection
  • January 15, 2027, at 11:30 a.m. — final approval hearing before Judge Ken Schubert, with remote attendance available

The settlement website lists only dates for these deadlines and does not state a time zone.

Current Status

Claims are open. Online claims are due by 11:59 p.m. Eastern Time on November 13, 2026, and mailed claims must be postmarked by that date. The court preliminarily approved the settlement on August 5, 2026, and the final approval hearing is scheduled for December 4, 2026. No payment date has been announced; payments depend on final approval and the completion of claims processing.

What Changed Recently?

Former Gritstone bio chief executive Andrew R. Allen agreed to a $6 million cash settlement, paid by his insurers, to resolve In re Gritstone bio, Inc. Securities Litigation in the U.S. District Court for the Northern District of California. The Stipulation of Settlement is dated May 15, 2026, and Judge Charles R. Breyer entered the order preliminarily approving the settlement and providing for notice on August 5, 2026. Strategic Claims Services is sending postcard notices and accepting claims.

The lead plaintiff alleges that Allen made materially false and misleading statements about Gritstone’s compliance with current Good Manufacturing Practices, its ability to meet its contract with the Biomedical Advanced Research and Development Authority (BARDA), and the timeline for launching the CORAL Phase 2b clinical trial, and that the stock price fell when the alleged truth emerged on March 1, 2024 and April 2, 2024. Allen denies all allegations of fault, liability, wrongdoing or damage, and says he acted properly at all times. The court has not ruled on the merits.

Gritstone bio, a vaccine and immunotherapy developer, filed for Chapter 11 bankruptcy protection in Delaware on October 10, 2024. Nasdaq suspended trading in its stock on October 22, 2024, after which it traded over the counter as GRTSQ until it was delisted on April 7, 2025.

Who Qualifies?

The settlement class is everyone who purchased publicly traded Gritstone bio securities between March 9, 2023 and April 2, 2024, both dates inclusive, and was allegedly damaged by doing so. In practice, the plan of allocation pays only on Gritstone common stock bought from March 9, 2023 through April 1, 2024 and held through at least one of the two alleged corrective disclosures.

Excluded are people and entities with no compensable loss; Allen and his immediate family; anyone who served as a control person, officer or director of Gritstone during the class period and their immediate families; Gritstone’s parents, subsidiaries, affiliates, successors and predecessors; entities Allen controls; trusts for his or his family’s benefit; his liability insurers; their legal representatives, heirs, successors and assigns; and anyone who submits a valid request for exclusion.

Shares from Gritstone’s April 2, 2024 offering of about 8.3 million shares are not eligible, and neither are shares acquired by exercising or converting non-publicly traded securities. Publicly traded options and warrants themselves do not qualify, though common stock acquired by exercising them during the class period is treated as a purchase.

How Much Can You Get?

Each authorized claimant receives a pro rata share of the net settlement fund — the $6 million plus interest, minus court-approved attorneys’ fees, expenses, notice and administration costs, and taxes. The lead plaintiff’s damages expert estimates about 54.9 million allegedly damaged shares; if all of them participate, the average recovery would be about $0.11 per share before deductions. That figure is an estimate, and actual payments depend on purchase and sale dates and prices and on the number and value of valid claims.

Current Status

Claims are open. The deadline to file is November 13, 2026, which is also the deadline to opt out or object. The settlement has preliminary approval, and the final approval hearing is set for December 23, 2026. No payment or voucher date had been announced as of September 28, 2026. If you paid a fee on an FTD.com or ProFlowers.com order during the class period, file a claim and choose cash or the voucher.

What Changed Recently?

FTD, LLC has agreed to settle Ciampi v. FTD, LLC, a class action in Florida state court in Miami-Dade County, and notices have gone out to customers the company identified. The plaintiffs allege that FTD advertised flower and gift prices on its websites without disclosing mandatory delivery fees, which they say appeared only at the final checkout step. FTD denies the allegations and does not admit any wrongdoing, and the court has not decided who is right. The court has certified the class for settlement purposes only.

Who Qualifies?

You are in the class if, while in the United States, you ordered flowers or other products for delivery on FTD.com or ProFlowers.com and were charged and paid a delivery, shipping, handling or service fee between January 29, 2021 and January 29, 2025. Those two sites are the only ones the settlement treats as the FTD Websites. If you received a notice, the administrator has already identified you as a likely class member.

How Much Can You Get?

Each class member who files a valid claim chooses one of two benefits: a $5 cash award, or a $15 voucher for use on FTD.com or ProFlowers.com. The benefit is fixed and does not grow with the number of orders you placed or the fees you paid.

The voucher comes with limits. It is good for one use, has no cash value, cannot be transferred and cannot be combined with other offers. It can be applied to taxes and fees, and it expires 12 months after it is issued.

Class Counsel expect to ask the court for up to $1,350,000 in attorneys’ fees and costs, plus service awards of up to $2,500 for each class representative. Those amounts and the cost of running the settlement are paid separately and do not reduce the $5 or $15 benefit.

What Proof or Notice ID Is Required?

To file online you need the Unique ID and PIN printed on the notice you received. Receipts are not required. A downloadable claim form is also on the settlement website for people who would rather submit a signed paper form; follow the instructions on the form.

What Is the Deadline?

Claims must be submitted online, emailed, or postmarked if mailed by November 13, 2026. The official notice does not give a time of day or timezone for the online deadline, so do not wait until the last day.

Requests to opt out and written objections are due by the same date and must be sent by mail. The notice says exclusion requests cannot be made by phone, fax or email.

How Do You Take Action?

Go to the official claim page, enter the Unique ID and PIN from your notice, and choose the $5 cash award or the $15 voucher. After you submit, you should get a confirmation email with a code; keep it in case you need to follow up. The settlement website also has the paper claim form and instructions for other ways to file.

What Happens Next?

The Honorable Migna Sanchez-Llorens of the Circuit Court of the Eleventh Judicial Circuit in and for Miami-Dade County will hold the final approval hearing at 11:30 a.m. on December 23, 2026, in a virtual courtroom. The court may change the date, and any change will be posted on the settlement website. No awards or vouchers go out until the court approves the settlement and any appeals are resolved.

The final approval hearing was scheduled for October 19, 2026 at 9:30 a.m. Central Time at the District Court for the Parish of Caddo in Shreveport, Louisiana. As of October 20, 2026, the official settlement website had not posted a final approval order or any ruling and still listed that hearing date, and no payment date had been announced.

What changed recently: the October 16, 2026 deadline to opt out or object has passed, and so has the October 19, 2026 hearing date, which fell nearly a month before the claim window shuts. Before the hearing, the settlement website posted class counsel's motion for final approval and application for attorneys' fees, costs and service awards. Class members who did not opt out are bound by the settlement if it is approved, and the claim deadline of November 16, 2026 is unchanged.

What Changed Recently?

The claim window opened this fall, less than a year after the attack. The claim form dates the incident precisely: it occurred on October 5, 2025, and OLHS identified it five days later, on October 10, 2025. The notice describes it as a targeted cyberattack on the computer systems of Regional Urology and Ochsner LSU Health — together referred to as OLHS throughout the settlement documents — during which certain files containing private information were accessed.

The categories of information named in the notice are the ones that matter most in a medical breach: name, Social Security number, date of birth, medical record number, and information related to care received at Regional Urology.

One detail in that description is easy to skim past and worth stopping on. The care information at issue is described as relating to care received at Regional Urology prior to December 31, 2022. The attack happened in October 2025, but the records it reached concern treatment from before the end of 2022 — so the people affected include former patients who may not have been to the practice in years, and who have no particular reason to be watching for a letter from it. A Social Security number and a date of birth do not expire; neither does the exposure.

OLHS denies that it did anything wrong, and the notice states plainly that the court has not decided who is right. The parties agreed to settle to avoid the costs, risks, disruptions and uncertainties of continuing the litigation. The case is captioned Clark v. Regional Urology, LLC, et al., Case No. 662345, in the District Court for the Parish of Caddo, Louisiana.

Who Qualifies?

The court defined the class in one sentence: all living individuals in the United States whose private information was potentially compromised in the data incident.

Two words in that definition do real work. Potentially is broad — it does not require anyone to show that their information was actually misused, or even that it was confirmed as accessed rather than exposed. Living is narrow, and it is the limit worth noticing: the class does not reach the estates of patients who have died, which on a practice whose exposed records date to 2022 and earlier is not a hypothetical exclusion.

In practice OLHS's records decide who is in. The notice tells its recipients directly that OLHS's records indicate they are class members entitled to benefits, and adds that they may have received an earlier notice from OLHS about the incident itself. Anyone unsure whether they are covered can ask the administrator through the settlement website's contact page.
